Kamu Hizmetlerinin Sunumunda Kamu Özel Ortaklığının Türkiye’de Sağlık Sektörü Öncülüğünde Uygulanabilirliği Üzerine Bir Değerlendirme

Sanayileşme süreci ve sonrasında devletin sunması gereken hizmetlerin hangileri olduğu sürekli tartışma konusu olmuştur. Kapitalizmin geçirdiği dönüşümlerle eşgüdümlü olarak devletin rolü sürekli değişmiştir. Minimal devlet anlayışından beşikten mezara kadar her türlü hizmetin kamu tarafından sunulması gerektiğini savunan sosyal devlet anlayışına kadar uzanan çeşitlilik, kamu hizmetlerinin neler olduğuna ilişkin yelpazenin ne kadar geniş olduğunu göstermektedir. Gerek sosyal devletin sürdürülemezliği gerekse küresel rekabetin ulus devletlerin vergileme konusunda elini kolunu bağlaması, devletleri kamu hizmeti sunarken daha dikkatli olmaya ve etkinliği göz önünde bulundurmaya yöneltmiştir. Kamu hizmetlerinde verimliliği artırma amaçlı birçok harcama ve gelir reformu yanında kamunun iş görme biçimlerinde de yeni arayışlara girdiği dikkat çekmektedir. Özelleştirme, yerelleştirme ve delegasyon gibi yöntemler yanında kamu hizmeti sunumunda birçok ülke, kamu özel sektör işbirliği yöntemleri geliştirmiştir. Böylece hem kamu bütçesinin topluma yükünü azaltmayı hem de özel sektörün rekabetçi ve verimlilikle ilgili genlerini kamuya aktarmayı başarmışlardır. Bu çalışmada küresel trendle uyumlu olarak sağlık hizmetlerinde etkinliğin artırılması amacıyla Türkiye’de geliştirilen yeni yöntemlerin başarısı ele alınmıştır. Yapılan düzenlemelerle hem devletin sağlık hizmeti sunumunda rekabetçi ve regülasyonu önceleyen yapılanmalara gittiği hem de özel sektörü sağlık hizmeti sunmaya daha fazla çekecek reformlar gerçekleştirdiği gözlenmiştir.

The industrialization process and the continuous discussion of which services the state should provide. In coordination with the transformations of capitalism, the role of the state has constantly changed. Ranging from minimal state understanding to the social state understanding, which advocates that all services must be offered by the public, shows how wide the range of public services is. Both the sustainability of the social state and the commitment of the nation states of global competition to taxation have led States to be more careful while providing public service and to consider efficiency. In addition to spending and income reform aimed at improving efficiency in public services, it is noted that the public is also looking for new ways of functioning. In addition to methods such as privatization, localization and Delegation, many countries have developed methods of public sector cooperation. Thus, they managed to reduce the burden of the public budget and to transfer the private sector's competitive and efficient genes to the public. In this study, the success of new methods developed in Turkey in order to increase the effectiveness of health services in line with the global trend has been discussed. It has been observed that with the arrangements made, both the state has gone to competitive and regulatory issues in the health service provision and has made further reforms to provide health services to the private sector.
